Carrington Medical Center recognizes that every competent adult has the right and responsibility to make the decisions relating to the adult’s own health care, including the decision to have health care provided, withheld, or withdrawn.
Advance Directives include Durable Power of Attorney for Health Care, Living Will and Health Care Directions. Advance Directives provide medical professionals guidelines to the extent of medical treatment you wish to receive. Advanced Directives are for health care issues only, it does not take the place of a regular will for financial matters.”
When signing in at the Admissions Office, you will be asked if you have Advance Directives and if you have provided us with a copy. If you do not have one, you may receive the packet of information and you may request to have the Social Worker explain the Advance Directives to you.
It is important this information remain in your medical record – if you have taken the time to complete an Advance Directive, it is important that your doctor know what decisions you have made.

There are several different types of Advance Directives, including:
- The Living Will "Do Not Resuscitate" order
- The Durable Health Care Power of Attorney Health Care Proxy
If you wish to prepare an Advance Directive, you should do so before you become too ill to think or communicate clearly. Advance Medical Directives are usually used to communicate your wishes regarding the withholding or removal of medical treatment.
